How an NHS Trust Used Integrated Data Sets to Advance Better Population Health Outcomes

Lewisham and Greenwich NHS Trust, responsible for two hospitals and several community health centers, realized the need to address disparities in health outcomes and unmet community health needs in 2017. The Trust moved on this need by investing in a population health and care management program that took advantage of HealthIntent, an integrated data set and platform. HealthIntent helped the systems identify and target vulnerable patient populations and reveal key gaps in care.

According to Hashum Mahmood, “The HealthIntent tool is a near real-time integrated dataset that allows end users to deep dive into patient population insights derived from data sourced from primary care, secondary care, community care and mental health. The tool has functionality that allows it to build queries to interrogate the data and measure impact of interventions. So far, PHCP projects are created in response to local queries from clinical directors and PCN leads to create products such as dashboards, registries and business object reports.”
