The Premier Meeting for Professionals in the Social Determinants of Health Space Driving Change in the Transition to Value-Based Care
The RISE National Summit on Social Determinants of Health is the place for cross-sectional thought leaders to discuss actionable, tactical, and scalable solutions to social determinants of health (SDoH) challenges and achieve better outcomes for the most vulnerable populations.
2020’s virtual conference will focus on four key themes: Cross-Sector Collaboration and Making Connections, Use of Data and Emerging Technologies, Funding and Reimbursement for SDoH Initiatives, and Insights into Policy and Legislature and the intersection of the Public / Private sector.
- Learn about recent successes and failures of community efforts to create outcomes-based and sustainable examples of solving for SDoH
- Walk away with ideas you can put to work and avoid ideas that don’t work
- Forge new relationships with other attendees to share and collaborate with those on the front-line solving SDoH
We look forward to convening next year and continuing to forge relationships among cross-sector partners to drive effective collaboration to solve for SDoH.
