An understandable tension may exist between payers responsible for ensuring prudent use of the health care budget and pharmaceutical companies and patients who could derive benefit from such high cost products. To date, much of the risk in product reimbursement has been borne by the payer; however, the appearance of new, very high cost drugs frequently supported by a weaker evidence base has increased the risk of the reimbursement decisions dramatically, with obvious consequences for decision-makers and ultimately taxpayers.
